WHAT TO TAKE WHEN SUBMITTING APPLICATION (FORM D-44)

Properly completed Form D-44 signed by parents/guardians
$33 dollars for the fee (cash, check, and certain cards)
Your social security number
Your original or copy of your birth certificate, or another valid document
Valid proof of residency documents
Your certificate of completion of driver’s education (if under 17 1/2)

WHAT TO TAKE WHEN TAKING THE DRIVING TEST

Important: Make sure that the vehicle that you’ll be taking the driving test in meets the DMV requirements

Signed permit
Proof of insurance for the vehicle that you will be taking the driving test in
A vehicle to take the test in
